Contract Administration Toolkit

The following links provide sample templates for various documents that may be required in the administration of contracts which are subject to the Builders Lien Act.

If you choose to use documents in other formats, please ensure all required information is included to avoid delays in contractor payments and holdback release.

Document Purpose
Contemplated Change Order Used for proposed changes to the scope of work due to City request, design changes, site conditions, or other changes
Change Order Used for agreed amendments due to changes to the scope of work due to City request, design changes, site conditions, or other changes
Statutory Declaration Contractor to submit with progress payments and final invoice for holdback release
Notice of Certification of Completion To be delivered and posted by payment certifier following issuance of Certificate of Completion as per the Builders Lien Act Section 7(4)
Certificate of Completion To be issued and delivered by payment certifier upon completion of the project as per the Builders Lien Act Section 7(4)
Builder's Lien Checklist Submit this checklist with holdback release payment request or invoice
